Hospital-at-home Integrated Care Programme for the management of disabling health crises in older patients: comparison with bed-based Intermediate Care

This study analysed the clinical impact of a home-based intermediate Care model in the Catalan health system, comparing it with usual bed-based care to check if the extended CGA-based hospital-at-home programme has an association with shorter stay and favourable clinical outcomes and if is needed to do studies to test this intervention to the whole Catalan integrated care system. 

Interventions and approaches to integrating HIV and mental health services: a systematic review

• Available literature on interventions integrating HIV and mental health services reveal that there is much diversity in the approaches adopted in combining treatment modalities; ranging from integration within a single facility, to multi-facility integration, and integrated care coordinated by non-physician case managers.

• Existing evidence, although limited, suggest that integrating HIV and mental health services may be linked to improved patient and service delivery outcomes in diverse settings.

• There is a need for higher quality and robustly designed studies to evaluate and compare integration models at different levels of service delivery in terms of long-term impact on patient outcomes and cost-effectiveness, particularly in low- and middle-income countries with high HIV and AIDS burden.

Individual determinants of integrated care for indigent patients: an experimental study in Brussels: Pierre Smith

In metropolitan area, socially deprived patients with chronic disease undergo the fragmentation of health and social care services because of their complex needs. Integration of health and social care would benefit these patientes, however, it is not feasible or appropriate to integrate all the services for all vulnerable patients. This study aims at identifying the indicidual determinants of deprived patients´preferences in integration of health and social care. 

Integrated care: learning between high-income, and low- and middle-income country health systems

Integration is a challenging concept to define, in part because of its multiple dimensions and varied scope: from integrated clinical care for individual patients to broader systems integration- or linkage-involving wide range of interconnected services. This commentary compares integrated care in high-and lower- income countries. Although contexts may differ significantly between these settings, there are many common features of how integration has been understood and common challenges in its implementation

SCIROCCO Exchange - Capacity-building support for integrated care

Integrated care is a recognised solution to address the challenge of ageing population. Hence, it is important to commit to opportunities that increase the readiness and capacity of regions to implement integrated care. Knowledge transfer has been greatly recognised as an effective enabler of capacity-building support. However, effective knowledge transfer and capacity-building support requires dedicated support and infrastructure to ensure that the flow of appropriate information and knowledge between adopting and transferring entities is tailored to the local context and maturity of the particular healthcare system seeking the support. This is a precondition to help health and care authorities to build their capacity for the successful transition towards integrated care.

Interrupted time series analyses on the effect on integrated care among the older population

Current health and social care systems in most European countries, are highly fragmented into specialist services, and poorly designed to provide health and social care for patients with multiple health problems and social needs. As some groups of patients might have greater difficulties navigating in a fragmented and divided system than others, current system also risks increasing inequalities in access and use of health and social care services. The aim of this study is to investigate if a comprehensive integrated care system perform better than ‘standard care’ in regards to emergency department visits, hospitalization for ambulatory care sensitive conditions (ACSC), costs and re-admissions as well as increase equity in health care among older people.

Maturity requirements of a Good Practice – Using the SCIROCCO tool to assess them

The SCIROCCO self-assessment tool is an online self- assessment tool with an objective to assess a region’s readiness for integrated care. It builds on the conceptual Maturity Model for Integrated Care developed by the B3 Action Group on Integrated Care of the European Innovation Partnership on Active and Healthy Ageing.

The SCIROCCO tool helps regions to:

• Understand the strengths and weaknesses of their regional context for integrated care  and inform national, regional and local policy-makers about potential areas of  improvement;
• Adopt and transfer integrated care good practices by identifying their maturity  requirements and requirements for the potential transferability and scaling-up;
• Facilitate multi-stakeholder dialogues on progress towards the implementation and  delivery of integrated care;
• Facilitate twinning and coaching activities that help regions and organisations to better  understand the local conditions that enable the successful deployment of integrated care.

Implementing integrated care for multi-morbidity: analysis of experiences in 17 European programmes

Many countries are experimenting with new models of care provision and numerous integrated care programmes have been established internationally. However, little information is available on how to implement integrated care. The aim of this study was to provide more in-depth insights in the implementation of integrated care for developers and managers of integrated care programmes, policy makers, health insurers, and researchers.

Skills building seminar: Capacity-building for implementing integrated care

Quality of Care Transitions: Older Adults Experiences in An Integrated Care Trust

The transition of care from hospital to home is a high-risk time for older adults. The Partners at Care Transitions (PACT) programme aims to improve safety and quality of care transitions. We aimed to test the feasibility of using the Partners at Care Transitions Measure (PACT-M) to evaluate older adults’ experiences of the transition from hospital to home in an Inner London Integrated Care Trust and to identify factors impacting transition quality.
